As i know, you can get it at late game.

If you are playing offline, you can't complete the quest at early game but however you will be invaded by NPCs later. That's why you can finish the quest. Just activate this quest before invasions. White-Faced Varre quest
you can also get to Mohgwyn Palace later in a secret area without the medal.
